Note that you might not have information for every heading every month. In that case just put "NA" in that spot so we know that you did not just forget about it. ("NA" is a common abbreviation for "Not Applicable")
Check with your District Counsellor if they want you to fill in the CLub Counsellor and Host Family information every month or only when they change.
You can cut and paste from other documents such as Word into the fields in this Form.
